What the consult form asks for

Name, phone number, email, procedure interest, and an optional scheduling message. That is enough to call you back from (561) 833-6880. Do not enter Social Security numbers, medical histories, medication lists, or upload images.

How that information is used

To return your call and schedule a visit. If a webhook is configured, the same fields are forwarded to the practice’s inbox tool. We do not sell form submissions. A honeypot field is used to ignore obvious spam bots.

Technical data

Hosting providers may log IP addresses and standard server data. This site does not ship an advertising pixel. Google Analytics loads only if the host sets NEXT_PUBLIC_GA_MEASUREMENT_ID.

Once you become a patient, your clinical information is kept in the practice record — not in this marketing form. Ask the office for the formal Notice of Privacy Practices at your visit.
